Metformin
Metformin is a biguanide derived historically from research on compounds related to Galega officinalis. It became a first-line therapy for type 2 diabetes because it lowers glucose effectively without usually causing hypoglycemia when used alone and has decades of clinical experience behind it.
How it works
Metformin accumulates particularly in the liver through organic cation transporters. Its mechanisms are complex and dose-dependent, but they include effects on hepatic energy metabolism, mitochondrial respiration and signaling pathways such as AMPK. The net clinical effect is reduced hepatic glucose production and improved insulin sensitivity.
Because AMPK, mTOR, autophagy and mitochondrial function are also central topics in aging biology, metformin became a major geroscience candidate. Laboratory studies show effects on nutrient sensing, inflammatory signaling and cellular stress responses, but these mechanisms alone cannot establish a longevity indication.
Animal and cellular evidence
Metformin extends lifespan in some model organisms and under some experimental conditions, but not uniformly. In worms and certain mouse studies it has improved survival or health-related measures, while other models show neutral or adverse effects depending on dose, diet and age at treatment. This supports a hormesis-like interpretation: a mild metabolic stress can be beneficial in one context and excessive in another.
Human evidence
Large observational datasets in people with diabetes have linked metformin use with lower cardiovascular events, some cancers and mortality compared with several alternative diabetes therapies. These studies are important but are vulnerable to confounding: metformin users differ from patients who require insulin or other drugs, and diabetes itself changes baseline risk.
Small mechanistic trials in people have documented changes in insulin sensitivity, gene expression, inflammation and metabolic markers. The unresolved question is whether people without diabetes gain clinically meaningful protection from age-related disease. That question requires randomized outcome trials rather than extrapolation from diabetic cohorts.
Cancer, brain and cardiovascular research
Research has examined possible anticancer mechanisms involving insulin/IGF signaling, AMPK and mTOR, as well as observational associations with colorectal and other cancers. Experimental work also explores neurodegeneration and vascular biology. These are active research fields; none currently justify describing metformin as a general cancer-prevention or dementia-prevention drug for healthy adults.
Side effects and monitoring
Gastrointestinal symptoms such as nausea, diarrhea and abdominal discomfort are common when treatment begins and often improve with slower titration, administration with food or extended-release formulations. Long-term use can reduce vitamin B12 absorption, so periodic assessment may be appropriate, especially when anemia or neuropathy is present.
Lactic acidosis is rare but serious and is mainly a concern when metformin accumulates in settings such as advanced kidney dysfunction or severe acute illness. Renal function therefore matters when prescribing and adjusting the drug.
Exercise and healthy aging
One unresolved issue is interaction with exercise adaptation. Because metformin dampens some anabolic and mitochondrial signals, studies have examined whether it can blunt improvements from resistance or aerobic training in some individuals. This is especially relevant if the target population is otherwise healthy and active rather than diabetic.
Bottom line
Metformin is a proven diabetes drug and an important experimental tool in geroscience. Its low cost, long safety record and effects on nutrient sensing make it attractive for aging research. But the evidence threshold for prescribing a drug to healthy people for decades is much higher than the threshold for treating diabetes. Until randomized clinical outcome data support that use, metformin should be regarded as a promising but unproven longevity intervention outside its established indications.
